What I collect. The details you type in this form: your name, contact details, birthdate, work, income, expenses, savings, debts, existing insurance, family details and goals. This is personal and financial information, and I treat it as sensitive.
Why I collect it. To prepare your SPINEancial Diagnosis, to contact you about it, and to discuss insurance options with you if you want that. Nothing in this form is a promise of coverage or approval of any application.
Who I am. My name is Harvey Espinosa. I work under the name Harvey Spiner as a Licensed Insurance Consultant of Pru Life UK. You can check that I am a licensed agent on the Insurance Commission's list: insurance.gov.ph/active-licensed-agents.
Who sees it. Only me, plus the Google tools I use to store it. I do not sell your information or share it with other businesses for their marketing. If you later apply for a plan, the details you give for the application go to Pru Life UK under its own privacy terms.
Where it is kept. In a private Google Sheet that only I can open. You can ask me to correct or delete your answers at any time.
How long. I keep your answers while we are in touch. If we do not work together, I delete them after 2 years. Where the law requires me to keep a record, I keep only that. You can ask me to delete them sooner.
Your rights. Under the Data Privacy Act of 2012 (Republic Act 10173), you may ask to see, correct or delete your information, or withdraw this consent. Message me at harvey@harveyspiner.com.
